Medical Coding & Billing Assistant

Slánú Lab (Beta)

Founder & Solo Engineer2026 – Present · Building for outpatient clinics

Slánú Lab is a bilingual medical coding and billing assistant for outpatient clinics. The clinic uploads its own fee book (Excel or CSV), a doctor pastes a medical note, and a deterministic matcher scores the note against the fee book to suggest procedure codes for human review — the clinic's fee book is the only source of truth, so no proprietary CPT dataset ships with the product.

Outpatient clinics — especially bilingual and Spanish-speaking practices — need help turning free-text medical notes into billable procedure codes without buying into a big EHR or a proprietary CPT dataset they can't audit. The suggestion has to be explainable, human-reviewable, and priced against the clinic's own fee book.

I designed and built the whole stack solo as a single Next.js 16 app — UI, route handlers, and Prisma access in one process — with all server logic isolated under `lib/server/*` so it can be lifted into a Fastify service later without rewriting it. Cognito + Amplify + aws-jwt-verify gate every API route; `clinicId` is the multi-tenant security boundary, checked in every query. Fee-book upload is alias-aware across English and Spanish column names, and rows upsert on `(clinicId, code)` so re-uploads refresh prices instead of duplicating. The suggestion engine is deterministic — same note in, same codes out — designed so an LLM step can layer on top of it without ever replacing it.

What I shipped on this project.

Bilingual fee-book upload (Excel / CSV) with EN/ES alias-aware column detection: code · description · price
Deterministic suggestion engine: normalize note, tokenize, score against fee-book description + aliases, return up to 8 ranked candidates with confidence + plain-text reason
Multi-tenant by clinicId as the security boundary — cross-clinic reads impossible by construction
AWS Cognito + Amplify (client) + aws-jwt-verify (server) with auto-provisioned Clinic on first sign-in
Server logic isolated under lib/server/* so it can lift into a standalone Fastify service without rewriting
Postgres on AWS RDS via Prisma, upsert-based fee-book refresh, encounter → billing-line workflow
Human-review-first UI: every suggestion carries a "Suggestions require human review" disclaimer
Bilingual (EN / ES) throughout via next-intl, locale-prefixed routes with a proxy for locale detection
